What we do:
The Social Care Worker – Rehabilitation Support is a key part of the Intermediate Care Team. Referrals into the team are made via a professional involved in the individuals care, for example, social work staff, a GP, Community nurse, or Physiotherapist.
The Intermediate Care Team provides a short term support and reablement service to increase an individual’s independence, reduce their stay in hospital or in a care home, and help avoid an unnecessary hospital stay. The team supports the Service User to identify achievable and realistic (SMART) goals and provides the support an individual needs to work towards those goals. The team also provides a Community Multifactorial Falls Risk Screening service.
What you'll do be doing:
As a Social Care Worker – Rehabilitation Support you will actively participate with the multidisciplinary team, service user and their families/carers, in a team approach to planning, development, implementation and evaluation of service user rehabilitation, in a variety of settings, including their own homes.
